Theresa Hernandez, APN/CNP, is a functional medicine Nurse Practitioner that emphasizes a healthy gut for a healthy life. Helping each patient to recognize their health potential is always her priority.
Theresa treats a variety of conditions, including: Anxiety, ADHD, Arthritis, Auto-immune conditions, Depression, Diabetes, Heart Disease, Hypertension, Irritable Bowel, Fatigue, Weight Loss, Thyroid / Adrenal Dysfunction, and Hormone Imbalance

Theresa Hernandez received her BS in Nursing from the University of Chicago in 2002 and continued her education with a Masters of Science in Nursing in 2006 while working in the pediatric ICU at Rush University Medical Center. She is currently completing her certification in Functional Medicine with the Institute for Functional Medicine.
Her previous clinical experience includes 8 years in a pediatric ICU unit tending to the needs of young patients. She also worked for 6 years as a practitioner in an allergy/immunology clinic. Theresa also taught clinical and didactic classes at Rush University for undergraduate nursing students. A nursing excellence award winner in 2006, she is also a published medical author.
Theresa believes that listening to her patients’ problems and concerns will tell her more than any physical exam ever will. She loves what she does because she gets to listen to people’s stories, validate their concerns, educate them on their health/illnesses, and encourage them to be an advocate for the own health – sharing a lot of laughter, smiles, and stories in the meantime.
Theresa’s knowledge of functional nutrition informs everything she does with her patients of all ages. She has an especially deep devotion to pediatric patients and young families who are looking for guidance with food allergies, autoimmune complaints and issues with detoxification.
Theresa and her husband have two small children and live an active, healthy lifestyle as a family.
